چکیده

Pancreatic carcinoma (PC) is an aggressive disease that represents the fifth cancer-related cause of death in Spain. Surgery is the only potentially curative treatment, but in about 80% of patients the tumor is unresectable at diagnosis. Due to its accessibility, safety and profitability, abdominal ultrasound is usually the first imaging technique to be performed when a PC is suspected. The main limitations of ultrasound in pancreatic assessment are the presence of bowel gas, obesity, operator dependency and low reproducibility. However, with experienced hands, ultrasound examination at the bedside can provide signs of advanced disease, such as ascites and/or hepatic metastasis. The incorporation of contrast-enhanced ultrasound (CEUS) into clinical practice has improved the accuracy of ultrasound in PC diagnosis. Moreover, CEUS is a technique with few adverse effects that can be used in patients with renal failure or iodinated contrast allergy and it is less expensive than CT and MRI.

Vascularisation Pattern of Chronic Pancreatitis Compared with Pancreatic Carcinoma: Results from Contrast-Enhanced Endoscopic Ultrasound

Discriminating between focal chronic pancreatitis and pancreatic cancer is always a challenge in clinical medicine. Contrast-enhanced endoscopic ultrasound using Doppler techniques can uniquely reveal different vascularisation patterns in pancreatic tissue alterated by chronic inflammatory processes and even allows a discrimination from pancreatic cancer. This paper will describe the basics of ...

Endoscopic Ultrasound in the Diagnosis and Staging of Pancreatic Cancer

Pancreatic cancer is one of the digestive cancers with the poorest prognosis, so an early and correct diagnosis is of utmost importance. With the development of new therapeutic options an accurate staging is essential. Endoscopic ultrasonography (EUS) has a major role in all stages of the management of these patients. EUS has a high accuracy in the diagnosis of pancreatic adenocarcinoma and the...

Endoscopic ultrasound in solid pancreatic masses--current state and review of the literature.

Some 25 years ago endoscopic ultrasound (EUS) was introduced in clinical practice for better visualization of pancreas. At the time of introduction EUS was superior to other methods in detection of pancreatic masses allowing tissue diagnosis by later introduced EUS-guided fine needle aspiration (FNA). During the time EUS was improved, electronic probes replaced mechanical probes adding ability ...

Pancreatic cancer imaging: the new role of endoscopic ultrasound.

Pancreatic cancer is the most deadly of all gastrointestinal malignancies and has a very poor prognosis. Unfortunately, most patients present late in the course of their disease and, at the time of diagnosis, only 10 to 25% of patients will be eligible for potentially curative resection. Efforts must be oriented towards an early diagnosis and towards reliably identifying patients who can really...

Clinical Role of Contrast-Enhanced Harmonic Endoscopic Ultrasound in Differentiating Solid Lesions of the Pancreas: A Single-Center Experience in Korea

BACKGROUND/AIMS The differential diagnosis of pancreatic solid lesions remains challenging. The aim of this study was to investigate the accuracy of contrast-enhanced harmonic endoscopic ultrasound (CEH-EUS) in differentiating pancreatic cancer from benign lesions. METHODS We prospectively evaluated 37 patients with pancreatic solid lesions. After intravenous injection of a contrast agent (So...
